Collaborate with the CATCH Sub-Board and Executive Steering Committees of the organization to carry out the mission and goals of the integrated care network
Work with committees of the organization and consultants on negotiating contracts with payers and providers
Work to expand the network of providers delivering evidence-based programs and integrated care
Collaborate with healthcare providers to establish clinical integration processes to ensure that eligible clients are referred for evidence-based programs and services
Build collaborative relationships with grantors, community partners, internal and external stakeholders and local agencies
Educate key stakeholders including physicians, healthcare providers, hospitals, and managed care organizations as it relates to CATCH
Develop program sustainability strategies to monitor CATCH's financial viability, including overseeing grant applications and fundraising efforts
Assess and improve the effectiveness of existing programs, ensuring they reflect current best practices
Participate in health and human service forums and coalitions to increase awareness of CATCH mission and impact; and stay abreast of trends and policy developments
Building and maintaining strong Community Care Hub Provider relations
Deliver ongoing provider oversight and reporting, ensuring each Hub member meets the highest quality, security, and HIPPA standards and all contract requirements. This may include supporting audits of existing plans and the development of contingency plans to ensure coverage of all contract-required geographies, languages/cultures, and specialties (e.g., mental health, homeless, disabled)
Document and report Hub member roster, performance, and identifying areas for improvement
Set the direction and image for CATCH through initiative and leadership
Drive cross-functional process improvements and best practices
Identify technology requirements and work plans for supporting the Community Care Hub
Present updates to the leadership team and stakeholders
Perform other duties as assigned

Experience and Qualifications:
Master’s degree in business, health administration, or social work is preferred, along with five years of relevant experience, or a combination of education and relevant experience.
Three plus years of administrative leadership experience, preferably in healthcare, social care, aging, or managed care.
Minimum of 3 years of experience with healthcare contracting and grant management.
At least 4 years of experience in a leadership role with direct reports.
Demonstrated experience with public policy and governmental decision-making.
Active knowledge of aging health and human service systems.
Demonstrated experience in ensuring diversity, equity, accessibility, and inclusion are incorporated into policies.
Highly self-motivated and adaptable while working collaboratively in a team environment.
Excellent oral and written communication, analytical, planning, and strategic thinking skills.
Proficiency in Microsoft Office Suite or similar software.
